Create custom forms for patient intake, consent, feedback, and pre-consultation questionnaires. Forms can be sent via WhatsApp and responses are linked to the patient record automatically.

Creating a Form

Click "+ New Form" and use the drag-and-drop builder. Add field types: text, number, dropdown, checkbox, date, file upload, and signature. Use templates for common forms like patient intake or consent.

Assigning to Patients

Send forms to individual patients or assign them to trigger automatically — for example, send a pre-consultation form when an appointment is booked, or a feedback form after a consultation is completed.

Viewing Responses

View responses in a table format or individually. Each response is linked to the patient's record. Doctors can review form data before the consultation starts.

Filter responses by date, form type, or patient for quick access

Analytics & Export

View aggregate analytics on form responses — completion rates, common answers, and response trends. Export all responses to CSV for external analysis.

Important

Consent forms require digital signature fields. Ensure your consent forms comply with local healthcare regulations before deploying them.

Tips

  • Keep forms short — patients are more likely to complete forms with fewer than 10 fields
  • Use conditional logic to show/hide fields based on previous answers
  • Set up auto-reminders for patients who haven't completed assigned forms
